Position Summary
As a Direct Support Professional (DSP), you will support individuals participating in Individual
Supports, Community-Based Supports, and Community Inclusion programs. Your role will focus
on delivering compassionate, person-centered care that encourages autonomy and empowers
individuals to reach their full potential in their residence or within their community. In addition,
you will be responsible for completing accurate, person-centered Medicaid documentation
during each shift to ensure services are in compliance with regulatory standards and reflect the
goals and preferences of each individual.

Key Responsibilities:
  •  Provide Personalized Support
    Deliver individualized, person-centered care based on each participant’s unique service plan.
  •  Support Daily Living Activities
    Assist with ADLs such as personal hygiene, grooming, toileting, dressing, meal prep, feeding, and medication administration (if applicable).
  •  Foster Social Engagement
    Participate in fun, community-based activities that promote inclusion, independence, and connection, indoors and out!
  •  Promote Independence
    Encourage self-determination and support life skills like personal care, cooking, and household responsibilities.
  •  Assist with Transportation & Mobility
    Help individuals access their community safely and confidently.
  •  Ensure a Safe & Respectful Environment
    Maintain a supportive atmosphere where dignity and safety come first.
  •  Document with Purpose
    Accurately complete Medicaid-required documentation, reflecting goals, outcomes, supports provided, progress, and any health or safety concerns.
  •  Collaborate & Communicate
    Work closely with families, team members, management, and stakeholders to ensure quality care and consistency.
  • Support Compliance
    Cooperate with all licensing and DDD inspections, inquiries, and investigations.

  • Excellent communication and teamwork skills.
  • Ability to follow care plans, assist with personal care, and document services accurately.
  • Valid driver’s license and ability to transport individuals (if required by the position).

Compensation: $20.00 - $27.00 per hour
